For cellular services, you can disconnect the entire service (in which case the phone number will stop working) or you can change the billing from the University to the individual.  You must include the individual's email address on the Request Item when changing the cellular financial responsibility to an individual.

A request must be submitted through the Telecom Catalog using the Disconnect an Existing Voice Service (Delete) form. Once you enter your phone number, a question will appear asking if this a request to disconnect service or change billing responsibility. Use the drop down to select change. A new box will appear for you to enter the users email address (it's ok to use a non Berkeley email).

The catalog is rights driven. If you encounter any errors while attempting to submit a request, this usually means you do not have rights to do so. In this case, find the person in your department who is familiar with and/or responsible for submitting these types of requests and have them submit the request for you.

Please use the following link to help guide you to the correct form.

Once submitted and assigned, we will contact the carrier to release the phone number. The user will then have 30 days to contact the carrier to assume responsibility for the phone number and either move it to a personal account, or port the number to another carrier. Failure to complete this action will result in the loss of service on day 30.
