Use the Add New Desktop Telephone Line Only form if you already have a UCB-purchased phone which you will use with a new line.
NOTE: We only support UCB-purchased equipment on our telephone switching equipment. If you already have a UCB-supported phone, you must provide us with the model number (example: Avaya 9404) to ensure your line is properly configured and billed.
If you wish to upgrade or downgrade a line, please use the Change Service form.
Additionally, on this form you can request to add a new digital PBX phone line for use with the Avaya Workplace VoIP service offering. This will provide a fully virtual line that is not terminated in an office and is not connected to a physical phone set.
