Transfer Cellular Service to UC Berkeley from another Carrier or Account

The following are steps for filling out the Telecom Catalog Add New Cellular Device and Plan form when transferring an established cellular service to UC Berkeley from another carrier and/or account.

 

Transferring a service that is currently active with a UC Berkeley offered Carrier (AT&T or Verizon only)

*Note: This request requires the user (or account owner if transferring from a business) to contact the carrier to initiate a 'transfer of billing responsibility' to UC Berkeley.

  1. Department Information
    1. Department ID
    2. Department Approver (if applicable)
  2. Cellular Options
    1. Choose your carrier
    2. Enter the device make and model you wish to purchase [if not purchasing a new device, enter N/A]
    3. Is this for a new cellular phone number? Auto-populates to YES. Use the drop-down menu to select No. A new box will appear to enter the cellular number you wish to transfer
    4. How many voice allowance minutes do you want per month? [Enter the desired voice plan. To view plan options visit https://technology.berkeley.edu/wireless]
    5. Would you like Insurance? AT&T does offer insurance. Verizon does not offer insurance.
    6. Do you call outside of the US?
    7. Will you be using your phone while traveling internationally? [Recommended for frequent international travelers. For casual international travelers, there is a Telecom Catalog form to request international service activation for specific international travel dates. See Cellular International Activation Request for more information.
    8. Do you want tethering? [also called Hotspot]
    9. Do you want to pick up the device or have it delivered to you? Auto-populates to our on-campus pickup location at Evans Hall.
      1. If you choose this option, the device will be delivered to Evans hall where it will be made available for pickup only
      2. If you would like the device delivered directly to the user, use the drop-down menu to select 'deliver to me'. A new box will appear to enter the delivery address. The address must accept FedEx and UPS deliveries. Home addresses are accepted (remember to include apartment or suite numbers). P.O. Boxes are not accepted.
  3. Billing Information [Department chart string]
  4. Order Contact Information [Who should we contact with questions about this order?]
  5. *Order Comment Box [Confirm that the user has released the phone number to UC Berkeley. Enter any additional comments]
  6. Add to Cart on the right hand side of the screen. Your order will populate in the cart toward the bottom of this section, a new button will appear: View and Edit cart
  7. Click View and Edit cart (you will be taken to a new page)
  8. Place Order in the upper right hand corner to submit.

 

Transferring a service from a carrier not offered by UC Berkeley

*Note: This request requires current account details

  1. Repeat steps 1-4 above
  2. *Order Comment Box [Enter 'Transferring service from (carrier name) to UC Berkeley'. Enter any additional comments.] Provide account details required for porting:
    1. Billing account number
    2. Billing account address
    3. Billing account PIN or Port PIN
    4. Social Security Number (required if transferring service to AT&T only)
  3. Repeat steps 6-8 above
