Process for installing a new line using a CUSTOM NAME as your Subscriber

Use this process when installing a new line to create a CUSTOM NAME for that line. This will help you identify important lines and prevent erroneous disconnects of service. Please follow the instructions below in the Telecom Catalog ( https://ucb.service-now.com ):  

 

Once you are logged into the Telecom Catalog (1) click Self Service in the left nav-bar, (2) click Service Catalog.





  Then click (3) Voice Services and select the new service you would like to install.

 


Select the new voice line service you would like. Start to fill out the form. When you get to the section (4) "This Services is for...", select (5)  "Other".  You will have to click the (6) Magnifying glass and another window will open up.

When the window opens select the (7) blue NEW button.



A window will open.  Add the (8) custom "Name" and the (9) "Department". Click 

(10) "Save" or "Submit". (Clicking 'Save' will keep you in the popup form. 'Submit' will save your changes and direct you back to the main form.)

 




The name will auto populate into the (11) "This Service is for" section in the "Other" name Field. 

Please complete the ADD service request form add it to your shopping cart and checkout. Once you have checked out you will receive a RITM number via email.
