Documentation of the patient creation process for Radiology/Urgent Care to ensure accurate registration and billing for Visitors and Reciprocity students.
1. Obtain legal documentation (Identification Card, Student ID, Driver’s license) to ensure accurate information is being used to register the patient.
2. Click ‘Registration’ to access Open Registration.
3. Confirm if patient is currently registered in PNC. Search for patient via legal name in the ‘Patient’ field and click the ‘search’ button.
Tip: Search for patients via ‘3, 3’ rule (first three initials of last name, first) to confirm if patient is currently registered in PNC: ‘War, Roa’ for patient Road Warrior

4. Users will see the following message if the patient has not been registered in PNC:
Select ‘Cancel’.
5. Select ‘New Patient’.
6. Complete the following fields on the General Tab:
· Last Name
· First Name
· Date of Birth
· Sex – Tip: do not leave as ‘Unknown’. This must be either ‘Male’ or ‘Female’.
· Patient # - Check the Auto-Gen checkbox.
· MRN - Check the Auto-Gen checkbox.
· Patient Type:
· Reciprocity Students – Students visiting from other UCs/other schools
· Visitors
7. Click the ‘Address’ tab to enter:
· Local Address
· City
· State
· Zip Code – standard 9 digit format (enter four 0s) i.e. ‘94720-0000’
· Local Phone
· Mobile Phone
Tip: Zip code MUST include 9 digits -a dd 0000 to zip code !

8. Click ‘Ok’ to save the patient demographics.
9. Select ‘Eligibility’.
10. Click ‘Add’.
11. Click Search:
· For Visitors, select ‘E1’.
· For Reciprocity, select ‘E2’.
12. Select the term and click ‘OK’ to save the eligibility information.
13. Select ‘Close’ to exit the Eligibility section.
14. Select ‘Insurance’ to add the insurance information to the patient’s account.
15. Select ‘Add’.
16. Select ‘INONE –No Insurance’.
17. Select the correct term and click ‘Ok’.
18. Plan information will now display in the insurance section. Click ‘Close’.
Patient’s Eligibility and Insurance will now display on the main registration screen. Patient Registration is now complete.
