Add New Cellular Service to an existing device

The following are steps for filling out the Telecom Catalog Add New Cellular Device and Plan form when ordering new cellular service for an existing device (does not need to purchase a new device)

 

*Requirements:

Note: The IMEI and ICCID (SIM card) numbers are found on the device by going to Settings>General>About for iPhones; Settings>About Phone (or about this device) for smartphones. ICCID (SIM card) numbers are sometimes rejected by the carrier, in which case a newly activated SIM card will be shipped to the user.

    1. Department ID
    2. Department Approver (if applicable)
  2. Cellular Options
    1. Choose your carrier
    2. Enter the device make and model you wish to purchase [Enter N/A or existing device]
    3. Is this for a new cellular phone number? Auto-populates to YES. Do not change
    4. How many voice allowance minutes do you want per month? [Enter the desired voice plan. To view plan options visit https://technology.berkeley.edu/wireless]
    5. Would you like Insurance? [Insurance cannot be added to pre-owned devices]
    6. Do you call outside of the US? [Applicable to AT&T customers only]
    7. Will you be using your phone while traveling internationally? [Recommended for frequent international travelers. For casual international travelers, there is a Telecom Catalog form to request international service activation for specific travel dates. See Cellular International Activation Request for more information.
    8. Do you want tethering? [also called Hotspot]
    9. Do you want to pick up the device or have it delivered to you? Auto-populates to our on-campus pickup location at Evans Hall.
      1. If you choose this option, the SIM card will be delivered to Evans hall where it will be made available for pickup only
      2. If you would like the SIM card delivered directly to the user, use the drop-down menu to select 'deliver to me'. A new box will appear to enter the delivery address. The address must accept FedEx and UPS deliveries. Home addresses are accepted (remember to include apartment or suite numbers). P.O. Boxes are not accepted.
  3. Billing Information [Department chart string]
  4. Order Contact Information [Who should we contact with questions about this order?]
  5. *Order Comment Box [Enter the IMEI and ICCID numbers. Enter any additional comments]
  6. Add to Cart on the right hand side of the screen. Your order will populate in the cart toward the bottom of this section, a new button will appear: View and Edit cart
  7. Click View and Edit cart (you will be taken to a new page)
  8. Place Order in the upper right hand corner to submit.
