Use the Cable Install Only (No Activation) / Cable Field Surveys, Cable Testing form in the Service Catalog when you want to request:

• Installation of data and/or voice cables (the cable that runs from a jack to a wiring closet), but do not want the cables activated; or
• Testing of existing cables;  or
• A cable field survey

To install and activate lines, do not use this form.  Use one of the following forms:

• To install and activate data network connections, use the Connect to Data Network form.
• To install and activate voice connections, select Voice Services on the Service Catalog and then select the appropriate form.
